Creating one Incident form and re-generating more forms to different department

I just have a quick question I hope I might get insight from this team.

Am creating workflows and Forms in the TOPdesk environment. I realized that one form can be used within two or three different departments. So for example a ticket is created for onboarding a new staff member. this form can be shared in IT (Maybe to create the user account and prepare the laptop) Then with HR (Maybe for the creation of ERP accounts and bank details of staff for payment). My question is, Is there a way to automate this form in a way that once the request or Incident form is submitted, in the background it splits into two different forms one goes to IT department, and another goes to Hr? Looking forward to your input on this.
